Do you have a loved one, friend, or neighbor you worry about because they are home alone during the day? Do you know an elderly person who would benefit from supervision and socialization? Caring for a loved one is serious commitment. And you do it with devotion. But you may need time away to work, shop, or attend appointments. The person you’re caring for may need a change of scenery, sometimes to be with friends and enjoy activities on their own. That’s why St. James Mercy is offering an adult day program called The Gathering Place.
What is Social Adult Day Care?
Social Adult Day Care is a program aimed at elderly or disabled adults who do not require ongoing intensive medical care but are in need of additional supportive services and supervision. This type of program also provides a respite or break for the caregiver. The goal is to provide the participants with services and support that ensure they start well and remain in their home while preserving their dignity and independence.
Services Offered
The program provides recreation for the participant, including social, spiritual, intellectual, and diversional (sewing, arts and crafts) activities designed to stimulate and enhance their functions.
The staff also provides personal care services to the participant such as ambulation, grooming, dressing, and hair care. A nutritional lunch is served daily with accommodations for special diets provided.
Facility Location and Hours
The Gathering Place is located in the Church Street Community Building at 71 Church Street in Hornell. There are 16 slots available in the program.
Hours of operation are Monday through Friday from 7:30am to 4:30pm.
However, flexibility will be maintained to accommodate family and participant requests. Each participant will have the opportunity to develop a schedule that best meets their individual needs, part or all of a day from one to five says a week.
Transportation
Transportation is included in the daily rate. The staff will arrange for transportation to and from the facility. However, the family may provide their own home transportation at a reduced daily rate.
Staffing
The Gathering Place is staffed by certified nursing assistants and activities specialists, with a registered nurse and case manager available if needed. Overall, there is a one-to-eight ratio in staffing to participants.
Eligibility
The participants must be 18 years or older and have an assessment completed by a case manager. An annual physical is required for all participants.
Program Cost
Individual insurance may or may not cover services. Private pay rates are affordable and flexible.
